Introduction
The AAH Healthy Living program requirements are set by AAH, not Foodsmart. So, only AAH can confirm a member's completion of required activities.
Foodsmart customer support cannot report a program or activity as completed to AAH members. Please contact the Healthy Living Solution Center at 1-800-937-5717, Monday through Friday, from 8 a.m. to 6 p.m.

Dear Foodsmart Participant,
Congratulations on selecting Foodsmart as your Healthy Living Support Program and taking important steps toward improving your health! The registration and program requirements below must be completed by the applicable deadlines in order to be eligible for the $250 portion of your Healthy Living Reward.
Please wait up to seven (7) business days from the date of this email to begin your registration with Foodsmart so that you will be recognized as an eligible Healthy Living participant.
Registration Requirements:

New Users:

Download the Foodsmart app to your mobile device.

Select Sign Up on the Foodsmart app or visit
https://zipongo.com/signup on your computer.

Enter your group: AAH Healthy Living.

Activate your free* account with your AAH Email (aah.org) and a password containing 8 to 15 characters with at least one number and one letter.

Enter your Healthy Living ID.

Your Healthy Living ID is the first initial of your first name, your last name and the last four digits of your Social Security number (e.g., jdoe1111).

Once you log in, a pop-up box will appear welcoming you to the 2024 Foodsmart program and asking you to click to proceed. Click on the button in the pop-up box to create your 2024 Foodsmart Account.

Schedule your first Telenutrition Visit.

To Schedule, select the Telenutrition button at the bottom of the home screen.

Select Schedule Appointment

Click Get Started and follow the prompts to schedule your initial visit.

After you schedule your appointment, you will receive a confirmation email with instructions on how to access your appointment and tips on how to prepare.

Returning Users:

Log into your existing Foodsmart account that you created in 2023.

A pop-up box will appear welcoming you to the 2024 Foodsmart program and asking you to click to proceed. Click on the button in the pop-up box to create your 2024 Foodsmart Account.

Schedule your first Telenutrition Visit.

To Schedule, select the Telenutrition button at the bottom of the home screen.

Select Schedule Appointment

Click Get Started and follow the prompts to schedule your initial visit.

After you schedule your appointment, you will receive a confirmation email with instructions on how to access your appointment and tips on how to prepare.

You must activate your 2024 Foodsmart account AND schedule your first Telenutrition Visit by July 26, 2024. Your registration is not complete until your first Telenutrion visit is scheduled.
Telenutrition Requirements:

To get started with the program, complete your first Telenutrition Visit.

To access your Telenutrition appointment, follow the instructions in your confirmation email.

During your first Telenutrition Visit, you will select your desired educational curriculum.
-
Curriculum choices include Healthy Weight Loss, Nutrition for Fitness, Gut Health, or Healthy Eating on a Budget.

Complete three (3) additional Telenutrition visits three to five weeks apart within a 16 consecutive week period.

You will complete a total of four (4) visits throughout your engagement.

Your 16 consecutive weeks of engagement begins when you complete your first Telenutrition Visit.

Once engagement has begun, it cannot be paused or restarted.

Complete all assigned curriculum work between your four (4) Telenutrition visits.
NutriQuiz Requirements:

Complete the NutriQuiz at least twice during your 16-week program engagement.

Your NutriQuiz must be completed before your second Telenutrition Visit
-
You will be required to complete the NutriQuiz again when you have completed your curriculum requirements.

Complete all above requirements by Nov. 15, 2024

When you complete the above requirements, your Registered Dietitian will provide you with a Certificate of Completion. You must obtain this Certificate of Completion by Nov. 15, 2024.
If you screened with a BMI ≥ 30, complete an onsite Follow-Up Screening and show one of the following: 5% weight loss, BMI < 30, or a body fat percentage within the healthy range for your age and gender to earn the remaining $150 portion of your Healthy Living Reward. You must complete your Follow-Up Screening onsite by Nov. 15, 2024.

*Advocate Health covers the $99.99 cost of your Foodsmart account. This will appear one-time as non-cash income on your paycheck (coded on your pay stub as "MISC INC"). The taxes will be applied to your paycheck the month after your registration is complete. If a spouse/partner is enrolled in the membership, the taxes will be applied to the medical plan holder's paycheck.
Program Requirements
2024 Program Requirements:

Members must register for an account through either downloading the app or visiting the webapp. They can sign in to their prior account and make sure to proceed through the registration button or create a new account if a new user. (THIS STEP MUST BE COMPLETE BEFORE THE END OF THEIR FIRST VISIT)

Members schedule their RD appointment

Members must complete:
-
FOUR RD visits (Four are required, and no more than 4 may be completed this year!)
-
Pick a program for 2024: Healthy Weight, Gut Health, Money Saving, Eating for Fitness (The program they choose MUST be documented at visit 1)

Complete a NQ prior to visit 2 and visit 4 (it is recommended that the RD complete the NQ with the member on the phone during visit 1 and 4)

Any assigned readings/homework as part of their curriculum between visits.

Scheduling: Members must participate for 12-16 weeks. It is recommended to schedule the 4 allowed/required appointments THREE weeks apart. This will ensure thee member meets the minimum 12 week participation requirement. It also provides a small buffer if an appointment is missed to allow members to still complete their program in the 16 week window.
Visits Limits: FOUR visits are allowed, and FOUR visits are required to complete the program. These visits must be completed within 12-16 weeks so please try to schedule visits ~3 weeks apart to ensure you are participating for the minimum of 12 weeks and to ensure that you are completing all four visits by week 16.
