1. If you are a Elevance employee, you should have received an email from Foodsmart with steps on how to sign up for the Elevance House Employee Pilot Program.
2. Click on Schedule Now
3. Create your Foodsmart account.
Once all the information is added, the green Next button will appear. Please click Next.
4. A verification text or email will be sent with you, depending on if you entered your email address or phone number above.
5. Enter your information. Once it is complete the Next button will be green.
6. You will then see the Coverage and Payment screen. You can now enter your Cigna credentials.
7. Your coverage will auto populate. It will then show you that you are participating in the Elevance: Food Benefit or the Elevance: Nutrition Counseling program.
8. You will then be prompted to choose a date & time that works for you for your first appointment.
9. Verify your appointment date and time. Enter a promo code if you have one.
10. Be sure to check Terms & Conditions prior to clicking on Schedule Visit. Once it is checked, the Schedule Visit button will turn green to allow you to book your appointment. Your appointment is not booked until you click on Schedule Visit.
11. The confirmation of your booked appointment will follow.
12. Choose to either Schedule Another appointment or Go to your dashboard to see your appointments.
13. If you choose to schedule another appointment or go to your dashboard.
To book another appointment, you will click on schedule another and start again at step 9.
If you go to your dashboard, you will see your appointments made.
14. You will get reminder emails and texts to the email and phone number we have on file to remind you of your upcoming appointments.
15. After you complete your first visit, you will receive your incentives for grocery money. To access it, login to your Foodsmart.com account. You will find your incentives above your scheduled appointments 48 hours after completing your appointment.
