This article guides you through registration for Foodsmart using a web browser as a CCHP BadgerCare Plus member.
Internet browser (mobile or desktop)
1. Click the following link, Chorus Community Health Plan | BadgerCare Plus | Website registration.
2. Sign up with your email address and BadgerCare Plus ID number (10 digits). You'll also need to create a secure password. Your password must be between 8-15 characters and contain at least one number and one letter. Once all the fields are filled out, click Sign Up.
3. For the best experience, download the Foodsmart app from the Google Play store or Apple App Store to your smartphone. Sign in with your newly created account credentials!
