Eligibility

  • Must be an employee or dependent with Corporate Cigna insurance
  • Group ID must equal 3174704

The number of visits and frequency is determined by your dietitian. Appointments are conducted by phone call or Zoom video


Scheduling

What to have ready:

  • Your Cigna Member ID and Group ID
  • Date of birth
  • Preferred appointment time and contact method

Step-by-step:

  2. 2Create a Foodsmart account or log in
  3. 3Enter your coverage details (Cigna Employee, Group ID, Member ID)
  4. 4Choose your preferred RD or let the system match you
  5. 5Select a date and time, then confirm your appointment

  • $15 Foodsmart Bucks for each of your first 5 completed visits
  • Maximum total: $75
  • Redeemable through Instacart via your Fresh Funds wallet

After creating your account, you can visit https://foodsmart.com/member/onboarding/sign-in to schedule appointments and view your Fresh Funds balance.
