Your Visit Benefits
- 1 initial visit (60 minutes)
- Up to 2 follow-up visits (30 minutes each)
- Appointments are conducted by audio call or Zoom video
- Visits are scheduled every 3 to 4 weeks or as clinically needed
Food Eligibility
Your RD completes a risk assessment during your initial visit. Your food eligibility is determined by your health plan, not Foodsmart.
How to Check Your Eligibility & Schedule Your Initial Appointment
Foodsmart verifies eligibility using your CenCal Member ID, Date of Birth and Full Name. When you contact Member Support, have all of these ready. If you show as ineligible and believe this is an error, contact Foodsmart Member Support for assistance.
Contact Foodsmart Member Support to schedule your initial appointment. A referral is created in Foodsmart's system after your eligibility is confirmed — this step must happen before your appointment can be completed.
How Your Food Referral Is Sent
- Your RD completes a risk assessment at your initial appointment
- Based on the assessment, a food referral is automatically generated
- Referrals are sent nightly to food delivery partners
- The food vendor will contact you to arrange delivery
